Survey fields

Four free survey field types turn any form into a survey:

• Star Rating — 3–10 stars, keyboard accessible.
• NPS — the standard 0–10 scale, scored automatically.
• Opinion Scale — numbered buttons with custom end labels.
• Tabular Survey — a rows × columns Likert grid, mobile-friendly.

Results appear on the Survey Results page with distribution and stacked-bar charts, an automatic NPS score (Promoter/Passive/Detractor), and a 30-day trend. Conditional logic works with every survey field — for example, show a follow-up only when NPS is 6 or less.
