How to Create an Order Form in WordPress

An order form lets customers tell you exactly what they want — product choices, quantities and contact details — without phone tag or scattered emails. Auto Form Builder captures the full order and customer information for free. The plugin does not process payments natively; you connect your payment provider via webhook when you are ready to collect payment.

What you'll need

Step 1: Install Auto Form Builder

Install the free plugin from WordPress.org and activate it from your WordPress dashboard. Open Auto Form Builder from the admin menu to begin. No e-commerce extension or paid tier is required to build an order form that captures product selections and customer details.

Step 2: Create a new order form

Click Create New Form and name it Product Order or similar. Plan your product catalog as dropdown or radio options before building — defined choices produce cleaner order data than free-text product names. Start with a blank canvas so the field layout matches your catalog structure.

Step 3: Add product, quantity and customer fields

Use a Dropdown or Radio Buttons field to list products, packages or service tiers visitors can order from. Add a Number field for quantity with min and max validation so fulfillment receives usable counts. Include Name, Email and optionally Address fields for shipping labels, invoices and order confirmations.

Step 4: Add conditional logic for options and add-ons

Show size, color, engraving or delivery options only when the relevant product is selected so simple orders stay short and complex orders stay complete. Conditional logic prevents customers from seeing irrelevant fields that slow down checkout-style flows. Test each product path to confirm required add-on fields appear when they should.

Step 5: Configure notifications — not built-in payment

Set admin email notifications so every order arrives in your inbox with full {submission_data} detail. Enable a user confirmation email summarizing what was ordered and explaining next steps for payment. Auto Form Builder does not process cards or checkout natively — payment happens in the tool you connect via webhook, such as Zapier, Make or GoHighLevel.

Step 6: Connect payment and fulfillment via webhook

Open webhook settings and enter your endpoint URL to push each order to your payment processor, inventory system or CRM the moment it submits. Enable optional HMAC-SHA256 signing if your receiving server verifies payload authenticity. Every order is also stored in your WordPress dashboard as a reliable record even if email or webhook delivery hiccups temporarily.

Tips

  • Be transparent on the form that payment is handled separately — tell customers they will receive a payment link or invoice after submitting so expectations are clear.
  • Use Dropdown fields for a fixed catalog and Radio Buttons when customers should see all options at once without opening a menu.
  • Review orders in the submission dashboard and export CSV for fulfillment batches — the dashboard is your source of truth when integrations need manual verification.
